Materials Needed: Post-it® Address Labels (3200-A), Post-it® Flag+ Ballpoint Pen, Black (692-P2BLACK), clipboard, spiral notebook for each student.

Description:
Observe students reading, drawing, working in a group, building, playing, testing, etc. throughout the day. Write observations on address labels with student’s initials and date in the corner. Flag observations that need to be followed up with an action plan. At the end of the day, stick informal observations of each student in the corresponding assessment spiral, take note of flags, and plan instruction accordingly!
Instructions:
Step 1: Place several sheets of Address Labels on a clipboard. Put ball point pen with flags with the clipboard.
Step 2: Write a student's initials and the date in the top right hand corner of a label, and an observation of that student.
Step 3: If the observation requires follow up or an action plan, flag it using the flags on the end of the pen.
Step 4: At the end of the day, place observation labels into the appropriate student's notebook. Take note of flags, and plan instruction accordingly.
Afterwards:
Objective: To observe, assess, and record student data informally and consistently throughout the day in an effort to differentiate instruction according to student\'s needs and progress.
Lesson Extensions: Students, parents, and administrators can see documented observations of behavior, struggles, and achievements in a clear and consistent manner.
Supporting All Learners: All students can be informally observed and assessed in this manner.
Assignment/Home Connection: Students can observe each other, the teacher, while reading, during experiments, etc. and jot down notes on labels which can then be transferred and placed in an appropriate folder/organizer/chart/etc.
